Instructions for filling up the form :
Keep the Demand Draft ready before filling up the Application forms. Kindly check the recruitment rules and eligibility criteria before filling application form.
The points marked as * are mandatory and needs to be filled by the applicant.
After duly filling up of the application form, the applicant is required to take out the printout, paste the photograph (same as uploaded on the application form), sign on the last page below the Declaration and mention the date. The hard copy along with requiste educational and experience certificates and demand draft should be sent to Dy. Head (HR), INSTITUTE OF LIVER & BILIARY SCIENCES, D-1, Vasant Kunj, New Delhi-110070 by 6.00PM on 16th July, 2012
There should be no variation between the details including the Demand Draft No. (hard copy) submitted and the Demand Draft No. entered on line.
In case, the hardcopy of the application along with the necessary documents and Demand draft is not received in the Institute by 16th July, 2012, your online application would not be considered for further processing.
The details of demand draft must match with the actual bank demand draft details submitted with the hardcopy.
The name of the applicant and post code must be written at the back side of demand draft.
