EQ: Degree in Pharmacy / Sr. secondary with Diploma in Pharmacy form a recognized Institution and qualified & registered as Pharmacist Under Pharmacy Act, 1948.
Candidates who wish to apply for any of the said posts advertised will have to remit the prescribed fees as indicated, by approaching any CBS enabled (Computarized) branch of State Bank of India across the country and request the Manager, State Bank of India to generate ESIC challan through Screen no. 7127 / 7128 and make the remittance in any of the 5 Power Jyothi Accounts ie., 31177956888 / 31177964973 / 31177965842 / 31177966120 / 31177966517 maintained at New Delhi main branch. They have to enclose the receipt copy of the challan obtained from the Bank along with their application. In case if they are not in a position to remit the fees through Power Jyothi Account they may remit application fee in the form of a Demand Draft issued by any scheduled bank in favour of “ESI Corporation, Account No. 1” payable at Bangalore. The Demand Draft of the fee should be issued on or after the date of publication of advertisement but before the last date for submission of Application Form. Fee payment in any other form will not be accepted.
